Using this Site—including requesting a “Free Discovery Visit,” “Call Back,” submitting forms, or downloading reports—does not establish a providerpatient relationship. A providerpatient relationship is only formed after you complete a formal clinical evaluation and sign required intake documents.
3. SERVICES AND BOOKINGS
We provide physical therapy services in St. George & Lehi, UT.
- Discovery Visits: Requests are subject to availability and are not guaranteed.
- Cancellations: Appointment bookings are subject to our clinic cancellation and noshow policies.
4. MOBILE MESSAGING TERMS (Required for A2P Compliance)
By providing your mobile number on our website, forms, chat, scheduling pages, or any other communication channel, you expressly consent to receive SMS/MMS messages from Align Therapy.
