  • Relevant Medical History: Possible R meniscus tear and mild osteoarthritis (OA) impacting walking, standing up from chair, crouching, and getting up in the morning

Presenting Problem

Pt reports a history of possible right knee meniscus tear and is wanting shockwave therapy to reduce pain to improve function as well as reduce or delay the need for possible surgery in the future. A recent x-ray showed mild OA in his right knee. This had been limiting function including walking, standing up from chair, crouching, and discomfort after getting up in the morning.

Assessment Findings

  • Past Medical History: OA R knee and L4/5 Disc herniation s/p 30 years
  • Objective Measurements: Right knee flexion is <130 degrees; hip range of motion is normal. Moderate right hip weakness. Knee strength is good.
  • Pain: Right knee pain with sit to stand in clinic
  • Diagnosis: Right knee pain likely caused by mechanical forces and osteoarthritis of right femoral condyle; negative tests for meniscus involvement

Treatment Plan

  • Long-Term Goal: Avoid surgery and learn long-term management exercises.
  • Short-Term Goal: Pain relief with activities of daily living
  • Frequency: Once a week for 5 weeks

The patient attended all prescribed sessions with follow up via text report at monthly intervals. 

  • Functional Improvement at 2 and 4 months from treatment: No pain with standing up from chair, walking, stairs, minimal discomfort with crouching, significant decrease in discomfort in the morning after getting out of bed
